
STEPS TO BECOMING A SUPERIOR LOGISTICS OHIO CONTRACTOR
STEP ONE – Register a Business Name
You will need to decide upon a business name and type of business, and set up a Limited Liability Company (aka, an LLC). You will then need to register your business.
You may contact your local County Clerk office/Secretary of State office for details on the process, associated fees, and ensure the chosen business name is not already in use. The County Clerk/Secretary of State will register your business name as an Assumed Name. Once completed and approved by the County Clerk/Secretary of State, you will be able to move forward to Step Two!
It is important to use the same business name that is registered throughout the process!
STEP TWO – Obtain an Employer Identification Number (EIN)
After you have registered your business name, proceed to www.irs.gov. On the home page, click on the link Apply for an Employer ID Number (EIN). You will need your registered business name from Step One for this!
STEP THREE – Obtain USDOT Number and Motor Carrier Authority Interstate Authority (MC Number)
After obtaining your EIN, go to www.fmcsa.dot.gov to obtain the USDOT Number for your newly registered business from Step One. On the left-hand side of the screen you click the option Get a DOT Number. On the next page, select the Get Started link for new registrants. You will then have a series of questions to answer
Once completed, you will be given a complete summary page with your DOT number and User ID. Please save and/or print this for future use!
After obtaining your USDOT number, you must wait approximately 48 hours to complete this step. Go to www.fmcsa.dot.gov/registration and click Get Authority to Operate (MC Number). Then, scroll down to Apply for Authority and click the link FMCSA’s legacy registration system to complete your application and pay the application fee.

You will then complete the necessary information for New or Additional Registration.
STEP FOUR – Obtain Insurance
The FMCSA requires proof of insurance to be filed with your company. You must also ensure that the agency you obtain insurance with will complete the necessary filing with the FMCSA. You will need insurance for your USDOT and MC Numbers to be activated!

STEP FIVE – Activate your USDOT and MC Numbers
Once you have obtained insurance, you will need to activate your USDOT and MC numbers with the FMCSA by filing a BOC-3 Form (Designation of Process Agent). You will need to obtain a process agent for this. An example of this agency can be found at www.processagents.com.
STEP SIX - Complete your W9
Follow the link here to download your Form W9, and then save and return it Superior Logistics Ohio.
STEP SEVEN- Complete your ACH Direct Deposit Authorization Form
Follow the link here to download your ACH Direct Deposit Form, and then save and return it Superior Logistics Ohio.
Once you have returned the Certificate of Insurance, Form W9, and ACH authorization form so Superior Logistics Ohio, you will be sent a Carrier Agreement to electronically sign.
